Ethelo enables you to create your own customized survey.
You can do this in 3 simple steps:
Step 1 - Create Question Groups:
Survey categories can be created by navigating to the Question Groups tab, under the "Survey" drop-down menu. Here you can group your questions for display or analysis.
Step 2 - Create Survey Questions:
Navigate to the Questions tab that can be found under the "Survey" drop-down menu. This page allows you to create your own custom survey questions and the type of answer responses required. Survey questions can be used to gather demographics or other information from your participants.
Survey questions are grouped in categories called "Question groups", which must be created (or selected from existing groups) in order to successfully create the survey questions.
The Questions tab can be found under the "Survey" drop-down menu in the Admin Panel. This page allows you to create your own custom survey questions and the format of possible answers. Survey questions can be used to gather demographics or other information from your participants.
Click the "Create" button to create a new Survey Question.
There are 5 types of Survey Questions and a "Header Text" that you can create:
- Single line: Allows a single line of text as a response
- Multiple lines: Allows you to specify the number of lines of text displayed. Participants may type more than the number of lines displayed
- Choose One (Radio): Allows you to list alternative options, where only one can be chosen.
- Choose Multiple (Checkbox): Allows you to list alternative options, where any number can be chosen.
- Slider: Allows you to specify a numeric range for the response, so the participant may chose a value within that range. You can set default values, and optionally, a "Snap" interval that increases or decreases the slider by a specific amount.
- "Header Text, no Question": This is used to specify text, which is often used to group or contextualize questions below.
Common Elements for survey questions:
For each survey question type, there are common elements.
- Title: The name you want to give the question, such as "How old are you?"
- Content: Additional information you may optionally wish to provide about the question
- Option to hide title on survey form
- Option to allow comments: Used when a discussion about the survey item is required
- Option to include in decision: Used for testing the decision prior to consultation
- Option to delete on publish
- Display Order: If you would like the Questions to be displayed in a particular order, then enter a number value in the "Display Order" column. Input the lowest number for the Question you would like to be displayed first and increasingly higher values for Questions you would like to sequentially display next.
Step 3 - Create a Survey Page:
The Survey Page is for creating your own customized survey page to be displayed in the web browser. On this page, you can edit the survey title, Url identifier and the background information participants will need before filling out your survey.
Customize Look and Feel of Survey Questions Using CSS
Customize the design of your Survey Questions by adding CSS code in the ‘Custom CSS’ area.
You can find the right CSS code for your customizations in our CSS Reference Doc.
Simply 'copy' the relevant CSS code from the document and 'paste' it in the 'Custom CSS' area, which you can find here: Appearance > Look and Feel > Settings > Custom CSS
